Every day, women in vulnerable communities face health challenges that could be prevented with better access to basic healthcare and education. In response to this urgent need, Health Horizons International launched Movimiento Mujeres, a pioneering initiative in the Dominican Republic that focuses on community health and chronic care through a gender lens. By equipping women community leaders with skills in primary care, prevention, health education, chronic care management, and the promotion of healthy lifestyles, we transform these women into health promoters, who are trusted members of their communities and help connect vulnerable populations to health and social services.
Our Challenges
In the Dominican Republic, gender disparities in health are pronounced, especially in rural and marginalized communities. Many women lack access to quality healthcare, face structural violence, and struggle with other social factors, such as poverty, limited education, and inadequate public services like clean water and sanitation. Undocumented individuals, particularly Haitian immigrant women, are especially vulnerable, often facing abuse and being denied healthcare due to their nationality, lack of health insurance, and barriers such as the absence of interpreters or bilingual doctors. The high costs of medication and treatments further add to the burden of chronic diseases among these women.
We address the need for accessible and quality healthcare by empowering women through education, community leadership, and advocacy. Our initiative aims to reduce the incidence of chronic diseases that disproportionately affect women, such as diabetes, hypertension, and cancer, and to provide support for mental health and violence prevention. Additionally, we advocate for the development of standards and protocols for improved approaches to noncommunicable diseases (NCDs) at the community level, as well as for inclusive and culturally sensitive public health laws.
Our Approach
We train women community leaders as Health Promoters, who become the frontline of health education and support within their communities. These Health Promoters conduct outreach to educate women and girls on adopting healthy habits, reducing high-risk behaviors, and understanding the importance of routine health check-ups, such as Pap smears and mammograms. By forming support groups, these leaders create spaces where women can learn about issues like domestic violence and mental health and connect them to necessary clinical services at community health centers.
Our initiative also includes leadership workshops that empower participants to become peer educators and multiplier agents, further extending our reach. These trained leaders educate other women and adolescents in their communities, helping to spread health knowledge widely.
Programs and Activities
We offer a range of programs designed to empower women with health knowledge and skills:

Reproductive and Sexual Health Education: We provide comprehensive education on family planning, contraception, pregnancy, childbirth, postpartum care, and reproductive rights. Additionally, we address sexual health by educating women about sexually transmitted infections (STIs), safe sex practices, and their rights to make informed decisions about their bodies.

Cancer and Menstrual Health Awareness: Our programs include education on breast and cervical cancer, teaching women how to perform self-examinations and understand the importance of regular screenings. We also work to break the stigma around menstruation by providing information on menstrual hygiene management and ensuring access to menstrual products.

Maternal and Child Health: Through workshops on prenatal and postnatal care, nutrition, and breastfeeding, we aim to improve maternal and infant health outcomes by supporting safe pregnancies and births, especially in underserved communities.

Mental Health and Wellness Support: We focus on mental health by providing resources and education on managing stress, anxiety, and depression, while promoting self-care and resilience. Our goal is to help women navigate mental health challenges in a supportive environment.

Chronic Disease Prevention and Management: We offer nutrition and fitness programs that teach women about healthy eating and the importance of physical activity, aiming to prevent chronic conditions such as obesity, diabetes, hypertension, and osteoporosis. We also provide education on managing these conditions, with a focus on medication adherence and regular medical check-ups.

Violence Prevention and Support Services: Acknowledging the widespread issue of domestic violence, we provide education on recognizing signs of abuse, understanding legal rights, and accessing support services. Our goal is to empower women to navigate these situations with confidence and the necessary support systems.
Our Impact
Success is not merely about numbers but about the tangible impact our initiatives have on the lives of women in rural communities of the Dominican Republic. True success is seen when women transition from being passive participants in our workshops to becoming vocal advocates for their health and communities.
As Olga Santos Castillo from Villa Emmanuel shared:
I encourage every woman to get a Pap smear. It’s crucial to be aware of our health, and while these tests can be costly, organizations like HHI are here to help us get the care we need.
Her words embody the transformation we strive for: empowering women to take charge of their health and inspiring others to do the same.
Success stories, such as that of Tata – a Haitian immigrant who discovered she had cervical cancer after attending an HHI workshop on routine check-ups – highlight the transformative impact of early intervention and accessible healthcare. Tata’s journey from fear and uncertainty to health and empowerment underscores the profound difference our program can make. Today, she passionately advocates for preventive care, using her story to inspire hope and courage in others.

“I had never seen a doctor, but the staff at HHI made me feel comfortable. I don’t speak Spanish, and yet, there was a person translating for me. I felt for the first time that I was a human being who was being listened to and valued.” Tata said.
